Fix cases where the server can return a FAILURE message with result code 0

Right now it sometimes happens that the dqlite server sends a FAILURE message that uses 0 for the result code, where 0 should really be reserved to indicate success. At least some of the guilty code is in gateway.c, but there may be other spots as well.
